NEW CLIENT INFORMATION

To become a new client at New Life, you will need to either use our REGISTER TO BE A CLIENT button/link on our Home Page, or you can call our office at 330.343.6600 to register as a client.  When you call, our office staff will get some basic information from you (Name, phone number, email address, insurance information, and a brief description as to why you are seeking services) and then they will send you a text message with a link to register as a client.  

 

Once you have registered, you are added to our practice as a client, and our intake department will then create a portal for you.  The intake department will load paperwork into your portal for you to complete and will send you an email with the information to access your portal.  ALL paperwork must be completed before our intake department can schedule you for an appointment with a clinician.  Once an appointment becomes available, our intake department will contact you to schedule you for two appointments, typically one week apart.  The first appointment is your intake appointment and the second is your follow-up appointment.
